ABSTRACT

Objective@#This systematic review and meta-analysis was undertaken to evaluate the efficacy of antibacterial photodynamic therapy (aPDT) in the treatment of periodontitis in type-2 diabetes mellitus (T2DM) patients and to provide better treatment for patients with T2DM complicated with periodontitis.@*Methods@#The PubMed, Cochrane, Embase, Web of Science, CNKI, CBM, and Wanfang databases were searched for relevant randomized controlled trials (RCTs). RevMan 5.3 was applied for the meta-analysis, and a systematic evaluation was conducted.@*Results@#A total of 8 RCTs were included. The results showed that compared to simple subgingival scaling and root planing (SRP), aPDT assisted SRP had a better effect on improving the probing depth (PD) at 3 months after treatment, The difference was statistically significant [WMD=-0.32,95%CI(-0.45, -0.2), P < 0.05], but 6 months after treatment, there was no significant difference in the two groups [ WMD=-0.15,95%CI(-0.40, 0.10),P=0.23]. During the 6-month follow-up period, there were no significant differences in the clinical attachment level (CAL), bleeding on probing (BOP) or hemoglobin A1c (HbA1c) between the two groups (P> 0.05).@*Conclusion@#aPDT-assisted periodontal nonsurgical treatment in T2DM patients can improve PD in the short term but has no significant effect on the improvement of CAL, BOP and HbA1c.

ABSTRACT

To systematically evaluate the efficacy and safety of external therapies of traditional Chinese medicine(TCM) combined with sodium hyaluronate(SH) injected in articular cavity therapy on knee osteoarthritis(KOA). The following databases such as CNKI, WanFang, VIP, CBM, PubMed and Medline were researched to collect the randomized controlled trails on external therapies of TCM combined with sodium hyaluronate injected in articular cavity therapy on KOA. The selection of studies, assessment of methodological quality and data extraction were performed independently by two researchers. The methodological quality was assessed by using the Cochrane system evaluation methodology and Meta-analysis were performed by using Cochrane Collaboration's the RevMan 5.3 software. Forteen studies involving 1 449 patients were included. All of the trails were not adequate enough in methodological quality. Meta-analysis indicated that compared with control group, external therapies of TCM combined with sodium hyaluronate injected in articular cavity could raise effectiv rate(<0.000 01) and cure-rate(<0.000 01), improve Lysholm score(=0.003) and reduce VAS score(<0.000 1). But two groups have no difference in Womac score (=0.13).Compared with the treatment with sodium hyaluronate injected in articular cavity, external therapies of TCM combined with sodium hyaluronate injected in articular cavity, a promising treatment options, can be complementary advantages, improve the clinical curativ effect. But it still needs low risk and high quality clinical trials to verify.

ABSTRACT

Objective To evaluate the efficacy and safety of early endoscopic retrograde cholangiopancreatography (ERCP) with endoscopic sphincterotomy (EST) in patients of acute biliary pancreatitis (ABP).Methods Databases including PubMed,EMBASE,Cochrane library,CBM,CNKI,WanFang data and VIP data were searched with " Endoscopic retrograde cholangiopancreatography" "Endoscopic sphincterotomy" "ERCP" "Pancreatitis" "Biliary pancreatitis" resection up to September 2017.Randomized controlled trails (RCTs) conternming the comparison of ERCP with EST versus conservative management in ABP patients were enrolled in the study.Two reviewers independently screened literature,extracted data,and assess the risk of bias of the included studies.The Meta-analysis was conducted by RevMan 5.3 software.Results A total of 10 RCTs involving 1 752 cases were included in the Metaanalysis.The results of Meta-analysis showed that compared with the control group,early ERCP with EST reduced the systemic complication rate of ABP (RR =0.66;95% CI:0.47-0.93;P < 0.05).However,there were no significant differences in overall mortality,local complication rate,and ERCP related complication rate in ABP between early and selected ERCP with EST treat (P > 0.05).Subgroup analyses indicated that early ERCP with EST therapy reduced the mortality of severe acute biliary pancreatitis (SABP),ABP with biliary obstruction and within 24 hours of admission (RR =0.48,0.35,0.25;95% CI:0.24-0.96,0.17-0.74,0.09-0.73;P < 0.05);decreased the local and systemic complication rate of SABP,ABP with biliary obstruction (RR =0.56,0.54,0.51,0.51;95% CI:0.37-0.86,0.33-0.91,0.32-0.81,0.32-0.80;P < 0.05);reduced the local and systemic complication rate of SABP (RR =0.42,0.47;95% CI:0.25-0.70,0.25-0.87;P <0.05) and shortened the hospital stay of SABP (MD =-11.22 d;95% CI:-16.09--6.36;P < 0.01).However,there were no significant differences of overall complication rate in ABP without biliary obstruction and hospital stay in MABP.Conclusion Early ERCP with EST therapy appears to be safe and effective for ABP patients.

ABSTRACT

OBJECTIVE:To evaluate the effectiveness and safety of repaglinide combined with metformin versus glimepiride combined with metformin in the treatment of type 2 diabetes mellitus (T2DM),and to provide evidence-based reference for the clinic. METHODS:Retrieved from CJFD, VIP, Wanfang database, PubMed, Embase, Medline and Cochrane Library, randomized controlled trials (RCTs) about therapeutic efficacy (HbA1c, FPG, 2 hPG) and safety (the incidence of ADR,hypoglycemia and gastrointestinal reaction)of repaglinide combined with metformin(trial group)versus glimepiride combined with metformin(control group)in the treatment of T2DM were collected. Meta-analysis was performed by using Rev Man 5.2 statistical software after data extraction and quality evaluation with Cochrane systematic evaluation manual. RESULTS:A total of 12 RCTs were included,involving 957 patients. Results of Meta-analysis showed that the decrease of 2 hPG in trial group was significantly better than control group,with statistical significance [MD=-0.70,95%CI(-1.02,-0.38),P<0.001]. There was no statistical significance in the decrease of HbA1c [MD=0.00,95%CI(-0.24,0.25),P=0.98] or FPG [MD=0.10,95%CI(-0.17,0.36),P=0.47],the incidence of ADR [OR=0.54,95%CI(0.28,1.06),P=0.07],hypoglycemia [OR=0.52,95%CI(0.13,2.06),P=0.35] or gastrointestinal reactions [OR=0.60,95%CI(0.15,2.41),P=0.47] between 2 groups. CONCLUSIONS:Repaglinide combined with metformin is better than glimepiride combined with metformin in reducing 2 hPG,but both of them have similar safety.

ABSTRACT

<p><b>OBJECTIVE</b>This study investigated the influence of Hawley retainer and vacuum-formed retainer on the articu-lation of Chinese speech of adult orthodontic patients.</p><p><b>METHODS</b>A total of 20 volunteers were recruited in this randomized controlled trial in accordance with the inclusion criteria. Participants were randomly distributed into two groups, namely, the Hawley retainer group and the vacuum-formed retainer group. Speech performance was evaluated at the following time inter-vals: before wearing retainers (T0), immediately after wearing both upper and lower retainers (T1), 24 h later (T2), 1 week later (T3), and 1 month later (T4). The phonetic parameters were acquired through the use of computer-based voice analysis software Praat version 5.4.21, and the final data were analyzed using SPSS 19.0.</p><p><b>RESULTS</b>Both retainers can influence the phonetic parameters of vowels /e/, /i/, and /ü/ and consonants /s/, /f/, /x/, /t/, /j/, and /q/. However, the Hawley retainer showed a more remarkable effect on the articulation of Chinese speech, and the distortion of /i/, /ü/, and /s/ sounds can last for more than 1 month. Nevertheless, the recovery period of the patients' phonetic function in the vacuum-formed retainer group was within 1 month.</p><p><b>CONCLUSIONS</b>Both retainers can induce distortion of some Chinese vowels and consonants in the speech of adult orthodontic patients, but the Hawley retainer can more significantly influence the articulation of Chinese speech than vacuum-formed retainer.

ABSTRACT

Objective To assess the effects of gastric bypass versus medical therapy in patients with obesity and type 2 diabetes. Methods The Cochrane library, Embase, PubMed, Chinese biomedical literature database and Wanfang database up to April 2014 were searched. Randomized controlled trails(RCTs) of frequently-used bariatric surgery for obese patients with type 2 diabetes were included. Study selection, data extraction, quality assessment, and data analyses were performed according to the Cochrane standards. Results Four RCTs involving 157 patients in the gastric bypass groups and 152 patients in the medical therapy group were enrolled. Compared with medical therapy, gastric bypass for type 2 diabetes significantly decreased the levels of HbA1C(mean difference = -1. 85% , 95% CI -2. 15 ~ -1. 56, P< 0. 01), fasting blood glucose( standard mean difference = - 0. 90 mmol/ L, 95% CI-1. 24 ~ -0. 57, P<0. 01), body weight(mean difference=-23. 39 kg, 95% CI -29. 17 ~ -17. 61, P<0. 01), waist circumference(mean difference= -15. 36 cm, 95% CI -17. 51 ~ -13. 22, P<0. 01) and the dose of hypoglycemic medicine; while it increased the number of patients with HbA1C<6% (RR=5. 49, 95% CI 2. 22 ~ 13. 56, P<0. 01), the rate of adverse events(RR=1. 96, 95% CI 1. 42 ~ 2. 70, P<0. 01), and the level of high-density lipoprotein-cholesterol(mean difference=1. 24 mmol/ L, 95% CI 0. 64 ~ 1. 84, P<0. 01). Conclusions Gastric bypass surgery is more effective compared with medical therapy alone for obese patients with type 2 diabetes. Further intensive RCTs of high-quality in multiple centers with long-term follow-up should be carried out to provide more reliable evidences.

ABSTRACT

Objective To objectively evaluate the intrusion effects on the maxillary incisors of implants versus conventional method(J-hook headgear ,utility arch) .Methods Such online databases as Cochrane library ,PubMed ,EMbase ,VIP ,Wanfang and CNKI were searched by randomized control test and prospective case controlled study about implants vs conventional anchorage for the maxillary incisor intrusion .After study selection ,assessment and data extraction conducted ,meta-analyses were performed by u-sing the RevMan 5 .1 software .Results Totally 11 studies were included ,6 studies involving 154 patients were included in the J-hook headgear group ,5 studies involving 127 patients were included in the utility arch group .The results of meta-analyses showed that :(1)Compared with the J-hook headgear ,implants had more decrement in the distance of upper center incisior cutting to palatal plane(P0 .05) .(2)Compared with the utility arch ,implants had more decrement in the distance of the center of resistance of the upper incisor to palatal plane (P=0 .010) ,less increase in U6-PP angle (P0 .05) .Conclusion The implants anchorage intruded the maxillary incisor was better than conventional method (J-hook headgear ,utility arch) ,it has more incisor intrusion ,shorter treatment time and more comfortable for the patients ,there was no difference in root resorption between the two groups .

ABSTRACT

Objective To investigate the clinical efficacy of Pingchuan cataplasm combined with hyperthermia therapy for stable chronic obstructive pulmonary disease (COPD) with qi deficiency syndrome of both lung and spleen. Methods Sixty patients of stable COPD with TCM syndrome of qi deficiency of both lung and spleen were selected and randomized into treatment group and control group, 30 cases in each group. Based on conventional treatment, the treatment group was treated by Pingchuan cataplasm combined with hyperthermia, and the control group was given analog stick applicator without additional heat treatment. After 6 weeks of treatment, the TCM symptom score and the time of the first exacerbation after treatment were observed. Results Compared with the control group, Pingchuan cataplasm can reduce the patients’ TCM symptom score (F=9.843, P=0.003), and extend the interval of acute exacerbation after treatment (F=4.451, P=0.040). Conclusion Pingchuan cataplasm combined with hyperthermia applicator can improve symptoms and reduce the frequency of acute exacerbation, thus can treat and prevent the acute exacerbation of COPD.
